Dr. DiBiase is a Disease Management Consultant involved in the clinical quality\, program development and clinician training. \nShe has practiced clinical cardiology and heart failure at Palo Alto Medical Foundation for 25 years. She has extensive experience in Disease Management and HF tele-management. She has been in active clinical practice at the Palo Alto Medical Foundation since 1990 and Medical Director of the Heart Failure Program since 1997. At the Foundation she has been involved in Quality Improvement and has done extensive physician and patient education. She is Board Certified in IM\, CV Medicine and Echocardiography. Dr. DiBiase attended Stanford University School of Medicine and completed training in Internal Medicine and Cardiovascular Medicine at Stanford.
